All name and/or address changes must be submitted to the Board office. This can be done by Name/Address change online form, fax to (225) 756-3472, or by mail to 18550 Highland Road, Suite B, Baton Rouge, LA 70809.

• Verification of Licensure:

If you need to obtain Verification of Licensure for another state, please submit this request in writing to the Board office with a $5.00 fee. Verifications are usually sent out within three to five business days and can only be sent via first class mail.

Verification of Licensure received by the Louisiana Board must contain a board seal.

• Duplicate Identification Card or Wall Certificate

If you wish to obtain a duplicate identification card, there is a $5.00 fee. Please submit requests in writing with fee attached. Duplicates are usually sent out within two to three business days.

If you wish to obtain a duplicate wall certificate, there is a $25.00 fee. Please submit requests in writing with fee attached. Duplicates certificates usually take six to eight weeks to process.

• Upgrade Request:

Upgrades may be requested after one has successfully completed the registration exam given through CDR. In order to upgrade, please must submit a letter requesting the upgrade, a $25.00 upgrade fee, and a copy of a current CDR card or the passing examination score.

Notice: Individuals must remain in supervision until the license is upgraded. Notification will be sent via first class mail.

• Licensure Reinstatement:

If you previously held licensure in the State of Louisiana and allowed your license to lapse, you must submit a notarized Application, $90 application fee if you are in Louisiana OR $115 application fee if out-of-state, a CURRENT copy of your CDR card, as well as Verification of Licensure from any state in which you have previously held licensure.
